Healthcare services are provided directly by a qualified medical team or indirectly through a system of referrals within the local medical community for eligible low to moderate income families in Angelina County.
Services include diagnosis and treatment, prescriptions, referrals for special needs, laboratory, health risk assessments, health education, preventive health, and chronic disease management, and other services as needed.
