Plan Review
A Plan Review is required for new or to-be-remodeled retail food establishments.

A plan review fee of $175 is required for each application.
> Mobile Facility Plan Review Application
A plan review fee of $175 is required for each application.
All plan reviews must include a floorplan, a copy of a menu, and $175 (cash or local check).

Annual Permits
Health Permit Renewal applications and fees due are sent annually via mail prior to expiration. Fiscal permitting renews September 1 each year. A forty-five (45) day grace period applies for payment of fees due. The permitted establishment will be subject to a $100 late fee for payment received after the forty-five (45) day grace period.
